Parakeratoris is the presence of nuclei in stratum corneum. It generally also has thinning or loss of the granular layer. It is non-specific, usually seen in diseases of increased cell turnover, whether inflammatory or neoplastic.

Differentials
In constrast, hyperkeratosis (pictured) may also show a heterogeneous stratum corneum, but a preserved granular layer is seen.
Main suspected contitions=
Consider at least the following:
Parakeratosis indicates indicating mainly actinic keratosis
If dysplasia covers all epidermal layers on skin, Squamous-cell carcinoma of the skin
