Cellular blood components for the following patients/situations should be irradiated to reduce the risk of transfusion associated graft-versus-host disease:[1]
- Hematopoietic stem cell (HSC) transplant recipients
- HSC transplant candidates under some circumstances
- HSC donors if allogeneic transfusion must be given prior to completing the harvest
- Congenital T-cell immune deficiency
- Intrauterine or neonatal exchange transfusion
- Transfusion from family members or HLA-selected donors
- Patients who have had fludarabine chemotherapy
- Hodgkin’s disease
- Leukemia
- Lymphoproliferative disorders
- Granulocyte transfusion
- Neonates
- Aggressive chemotherapy, radiotherapy or T-cell immunosuppression
